Passport Services
Chester County Library processes passport applications by appointment only.
Individual appointments should be scheduled online.
Family/Group appointments for 4 or more people should call the library (610-344-5957).
All those applying for a passport must appear in person at the appointment, including minors.
Passport photographs are available at time of the appointment for $15 per person (plus tax).
This applies only to those eligible to use form DS-11. We do not process form DS-82, which is mailed in directly by the applicant or submitted online.
We reserve the right to cancel or reschedule an appointment if the applicant arrives 15+ minutes late, without their forms filled out, or without required documents. We may also reschedule appointments if the library has an unplanned closure.
Passport Appointment Hours
- Monday – Thursday, 4:00 p.m. – 7:00 p.m.
- Friday, 10:00 a.m. – 12:00 p.m.; 2:00 – 4:00 p.m.
This facility accepts DS-11 appointments only.
This includes:
- First time U.S. passport applicants
- Applicants under the age of 16
- Applicants whose last passport was issued under the age of 16
- Applicants whose passport was lost, stolen, damaged, or issued more than 15 years ago
If none of the above statements apply to you, you are eligible to renew by mail using Form DS-82. Visit this webpage from the U.S. Department of State for renewal information.
Please bring ALL of the following to your appointment:
- A completed and unsigned DS-11 form for each applicant (use black ink);
- Additional forms may be required
- Proof of identity:
- Government-issued photo identification document. (Must be the physical ID – not photo or digital copies accepted)
- Proof of U.S. Citizenship:
- Fully valid, undamaged U.S. passport expired 5 years or more; will be submitted with the application OR:
- U.S. birth certificate (Original or Certified Copy) that meets the following requirements:
- Issued by the city, county, or state of birth
- Lists applicant’s full name, date of birth, and place of birth
- Lists both parents’ full names
- Has the signature of the city, county, or state registrar
- Has the date filed with the registrar’s office (must be within one year of birth)
- Has the seal of issuing authority
- Passport photograph (photos are available at the time of the appointment for $15 per person (plus tax))
When children (under age 16) are applying for a passport:
- All passport applicants under 16 must apply in person using Form DS-11.
- Both parents should be present.
- If one parent/guardian cannot go with the child to apply for the passport, they can give permission by completing and submitting Form DS-3053 “Statement of Consent” with the application. A photocopy of a government-issued ID of the non-attending parent (e.g., front and back of a valid driver’s license).
- If one parent has sole legal custody, legal documentation such as Court Order must be submitted.
- If one parent is deceased, a valid, original death certificate must be submitted.
- A certified U.S. birth certificate is required for all child applicants.

When teens aged 16 and 17 years old are applying for a passport:
- All passport applicants aged 16 and 17 must apply in person using Form DS-11.
- At least one parent/legal guardian must accompany applicants aged 16 and 17 to show parental awareness.
- Proof of identity:
- A learner’s permit, a junior driver’s license or valid school photo ID (physical IDs only; no photocopies accepted).
- A valid U.S. passport may be used as either proof of identity or citizenship but not both (an expired passport cannot be used as proof of identity).
- Proof of citizenship:
- A valid or expired U.S. passport OR:
- U.S. birth certificate (see above).
Payments
Two payments are required. Payment to the government and payment to the library must be paid separately.
- U.S. Department of State application fees: Pay by check (personal, certified, cashier’s, traveler’s) or money order payable to “U.S. Department of State.”
- Bring separate checks for each applicant per U.S. Department of State requirements. Current fee tables.
- Library fees: (payable by cash, credit card, or check made out to “Chester County Library”)
- $35 execution fee per applicant (no tax)
- $15 photo fee per applicant (plus tax)
- Library fees can be combined into one payment
